Getting to the Diagnostic Center
Our main office is located at 4911 Executive Drive, Peoria, Illinois.
We are located on the second floor of the OSF Saint Francis PromptCare North Building.
Our main office is not in OSF Saint Francis Medical Center.

Necessary Forms to Bring
Please bring insurance information or current medical cards and driver's license with you to the appointment. The information that follows should answer many of your questions about your appointment. Do not hesitate to call with any questions and/or concerns that are not addressed below.

What to Expect
Bringing children and infants with you is discouraged as appointments can be lengthy and patient-physician discussions can be complex. If children will be coming with you, someone else must accompany you to watch the children during your appointment. Your visit will usually take 1-2 hours. Unless otherwise instructed, there is no special preparation for your appointments.

In the Maternal-Fetal Diagnostic Center, a nurse will complete a history form about your current and past pregnancies, medical and surgical information, and the family health histories of both you and the baby's father. It may be helpful if you prepare a list of this information and any questions/concerns before coming for your appointment. Please bring along all medications you are presently taking and a list of any allergies you have.

The doctor who referred you has already been asked to send copies of all appropriate medical records, laboratory tests, and x-ray and sonogram reports. However, if you see any other doctors, please obtain copies of their records and bring them with you. The more information you and your doctor provide, the more timely, thorough, and complete your visit will be.

After completing your history information, a complete ultrasound (sonogram) examination will be done. This examination will be performed by a registered diagnostic sonographer (RDMS) and a maternal-fetal medicine subspecialists (a physician whose specialty is high-risk pregnancies). Services of the maternal-fetal medicine subspecialist include ultrasound performance, consultation, interpretation/diagnosis, recommendations, and related testing procedures.

Payment
Co-pays are expected on the day of the visit. Separate billings will be submitted for physician services by the physician you will see - Maternal-Fetal Medicine Associates, S.C., and from OSF Saint Francis Medical Center for the ultrasound.

Insurance
It is not possible for us to know what your individual insurance policy covers. It is important and advisable that you contact your insurance carrier regarding your coverage prior to your visit. Confidentiality: The information in your medical record is confidential. Other than your referring doctor, we will not divulge any information without your written permission.

I-80, Princeton, LaSalle/Peru, Ottawa

From the North: Take Route 29 South from the Chillicothe area. Take Route
6 South to Peoria to Route 40 (Exit #6 Knoxville Ave. Turn right, South onto Knoxville Ave. The first stop light past Proctor Hospital is Glen Ave. Turn right onto Glen. Go to the second stoplight which is Isabell. Turn right onto Isabell.
The building is on your right.

Canton
From the South: Take Route 24 North to I-474 West to the Peoria exit (East I-74). Take I-74 to Exit 88 (Sterling Ave.), turn left onto Sterling and go 8 red lights to Isabell (Sterling Ave. becomes Glen Ave. as you pass War Memorial Drive). Turn left onto Isabell. The building is on your right.

Bloomington, Champaign/Urbana
From the East: Take I-74 West to Peoria to Exit 88 (Sterling Ave.). Turn right on to Sterling and go 7 red lights to Isabell (Sterling Ave. becomes Glen Ave. as you pass War Memorial Drive). Turn left onto Isabell. The building is on your right.

Quad Cities/Galesburg
From the West. Take I-74 East, Exit 88 (Sterling Ave.), turn left onto Sterling and go 8 red lights to Isabell (Sterling Ave. becomes Glen Ave. as you pass War Memorial Drive). Turn Left onto Isabell. The building is on your right.
