Instructions for completing application to the School of Radiography

1. Download, print out, and complete the application form.
2. Print out three (3) Reference forms and distribute to the three people you list on the application for references. If you sign the waiver on the reference form, you must have the referring person send the form directly to the address below. If you elect not to sign the form, you may send in the completed references with your other material.
3. When you submit your application, enclose a $50 nonrefundable application fee made payable to: School of Radiography.
4. Have all high school and college transcripts and ACT scores mailed directly to the School of Radiography. (ACT scores are usually on high school transcripts, provided you took the exam at that time.)
